Our Policies

 

Payment Policy [effective 1/2/2021]

Hope Connection strives to ensure a clear understanding of your financial responsibility with respect to the hyperbaric services we provide. These policies apply to all procedures and departments. Pay-as-you-go payments: Payment is due prior to each session by check, cash, or through credit card payment.

Bulk package purchases: We require payment in full of for all bulk-hour package purchases prior to the first session. Bulk package pricing offers client-patients a discounted per-hour session rate based on the total number of hours purchased, in accordance with our current pricing schedule (see https://hope-connection.com/prices.pdf). In general, client-patients may purchase session packages in 10-hour increments to receive discounted per-hour pricing; package pricing enables us to provide affordable hyperbaric oxygen therapy pricing to our client-patients.

Financing: We do not offer financing options at this time, but financing through a credit card provider is accepted. Hope Connection reserves the right to refuse treatment at any time. Insurance:

Insurance: Hope Connection does not accept insurance (private insurance, Medicare, or Medicaid) but our billing department will provide any needed payment transaction summaries to help with employer-defined benefits programs.

Late/Missed appointments: Missed appointments or appointments where 48 hours notice is not provided may result in a $50 rescheduling fee due prior to scheduling of the next session.

Payments: We accept cash, Visa, MasterCard, Discover and Care Credit, as well as payments by check and debit card. We may hold a credit card number on file to reserve an appointment, but only with your express permission. Hope Connection will send client-patient accounts to collections for balances not paid after receipt of two statements, unless payment arrangements with our billing department are made. We reserve the right to require payment for services to be made at or before the time of service.

Outstanding balances: We may refuse to see patients with balances over $250 and who are not making regular payments on the balance. If you have an unpaid balance at the end of a billing cycle, we apply a $15 late payment fee to balances. If you make a payment and it is insufficient to pay both the late payment charge and the principle amount due, we apply your payment to the late payment fee due and then we apply the remaining amount to the principal. In the event that your account is placed in collection, a collection fee will be added to your account, along with any attorney fees and/or court costs that may be necessary for recovery of the outstanding balance. In the event of an NSF check, there will be a $30 NSF charge added to the balance due.

Cancelations without sufficient notice: We will charge your credit card on file if you do not call and cancel your appointment within 48 hours. Regular appointments: We charge $25 to your credit card, if you do not call and cancel your appointment 48 hours ahead of time for all regular scheduled appointments. Working with your insurance provider or employer: We happily provide you with all payment transaction history and will write letters to any agency (state, federal, or any non-profit organization), to assist you in applying for grants for your hyperbaric oxygen treatment(s), as a courtesy.

Please keep in mind that payment remains your responsibility. We do not enter disputes over insurance benefits, and for any type of insurance arrangement Hope is engaged in, we expect payment in full from you if your insurance company or other agency delays processing of your claim. You agree to pay any portion of the charges not covered by insurance, grants, or any other non-profit entity working on your behalf. In cases where a written court order allows payment for medical costs associated with a dependent, it is your responsibility to obtain reimbursement from the other party involved.

Forms/Letters/Medical records: We may bill $25 for forms or letters that a provider requests Hope to complete on your behalf, and we may charge a $25 copy fee for medical records requested for personal use.

2021 Pricing Schedule

Refund Policy

Thank you for purchasing your oxygen therapy package or hyperbaric oxygen chamber (or rental) through Hope-Connection.

We offer full refunds on all purchases within the first 15 days after purchase, and partial refunds on all bulk session-package purchases made on our website, based on the number of hours purchased multiplied by the current pay-as-you-go rate (which is currently $200). If you are not satisfied with the product(s) or the package pricing that you have purchased from us, you will receive a refund on your credit card, no questions asked, within 15 days of your purchase. To be clear, you are eligible for a full reimbursement within 15 calendar days of your purchase.

We encourage anyone who is interested in using hyperbaric oxygen – either at our facility, or at home – to identify the minimum number of hours that would be needed to allow the client-patient to make a thorough attempt at improving the symptoms that the client-patient is using hyperbaric oxygen for to improve his/her health. We encourage all customers to try our home-unit (portable) hyperbaric oxygen therapy (HBOT) chambers for one week before finalizing a two-, three-, or four-month rental agreement with us. Our primary goal at Hope Connection is to identify whether hyperbarics – either at home or in-house at Hope’s office – is suitable for the client-patient and his or her symptoms identified on the client-patient’s Intake Form.
